It was brought to my attention that you still would still like to use the fitness center with your outside trainer. I do want to remind you that Mr. Blanc's application still needs to be submitted for approval.



We do have the application that has been signed already on our files since February but it is still pending to be submitted for approval due to the lack of some documentation which are the followings:



Copy of current Personal Training Certification.
Up to date Certificate of Liability Insurance naming
Copy of current CPR/AED Card.


Once we have all this documention the application will be submited for approval.
